What Is Medullary Sponge Kidney?

Medullary sponge kidney, sometimes called sponge kidney, is a congenital condition involving small cystic changes in the collecting ducts within the renal medulla. Some people have no symptoms, while others develop kidney stones, blood in the urine or recurrent urinary tract infections.

What Can Ultrasound Show?

Ultrasound can demonstrate kidney stones and calcium deposits associated with medullary sponge kidney.

A retrospective study published in BMC Nephrology reported a combination of ultrasound features: hypoechoic medullary areas, bright echogenic spots, small cystic dilatations in the papillary region and multiple papillary calcifications. These findings can support suspicion of medullary sponge kidney, but should not be treated as a standalone diagnostic checklist.

Can Ultrasound Confirm Medullary Sponge Kidney?

Ultrasound findings need to be interpreted alongside medical history, laboratory results and other imaging when appropriate. A normal or inconclusive ultrasound does not necessarily exclude the condition.

NIDDK describes several imaging methods used in the diagnostic evaluation, including ultrasound and CT. The appropriate investigation depends on the clinical situation.

Is Medullary Sponge Kidney the Same as Nephrocalcinosis?

No. Medullary sponge kidney describes a structural abnormality of the collecting ducts. Nephrocalcinosis describes calcium deposition in kidney tissue.

They may occur together, but identifying medullary calcification does not by itself establish medullary sponge kidney. The distribution of findings and the broader clinical evaluation matter.
